Consent to the Processing of Website User’s Personal Dat

By filling out the contact form on the website nvhealth.ru and checking the appropriate box, I hereby freely, voluntarily, and in my own interest provide my consent to Health Navigator Joint-Stock Company (hereinafter referred to as the ‘Operator’) to process my personal data.
This consent is granted in accordance with Federal Law No. 152-FZ dated 27 July 2006 ‘On Personal Data’.
1. Personal data subject to processing
The Operator may process the following personal data that I provided when completing the contact form:

• Full name
• Phone number
• Email address
• Information contained in the text of the user’s inquiry

2. Purposes of personal data processing
The Operator processes personal data for the following purposes:

• To identify the user who submitted an inquiry via the contact form
• To review the user’s inquiry
• To communicate with the user
• To provide the user with information, as well as consulting, informational, and technical support

3. Actions with personal data
The Operator may perform the following actions with respect to personal data:
collection, recording, systematization, accumulation, storage, clarification, updating, modification, retrieval, use, anonymization, blocking, deletion, and destruction of personal data.
Personal data may be processed either with or without the use of automation tools.
4. Transfer of personal data
The Operator shall not disseminate the user’s personal data.
Personal data may only be transferred to third parties in cases specified by the laws of the Russian Federation. Personal data may also be transferred to parties hired by the Operator to support the operation of the website and the receipt, storage, and processing of user inquiries, provided that such parties observe the confidentiality and security of the personal data.
5. Validity period of consent
This consent shall remain valid until the purposes of personal data processing have been achieved, but for no longer than the time required to review an inquiry and communicate with the user, or until the consent is withdrawn, unless otherwise specified by the laws of the Russian Federation.
6. Withdrawal of consent
I have been informed that I may withdraw this consent by sending a corresponding request to the Operator at the email address info@nvhealth.ru with the subject line “Withdrawal of consent to the processing of personal data”.
